What follows are electronic versions of the tools in your Fifth Grade Data Notebook. You can use these to tweak them to fit your class, or so you or your students can fill them out electronically for an electronic portfolio. Also, feel free to visit other grade level sights to see what they are measuring or to get some inspiration!

Fluency Graphs

Included are three forms for tracking reading fluency – Fry’s sight words, Fry’s Instant Phrases, and Reading Passages.

Students can quiz each other regularly and track the number of sight words, Instant Phrases, or words per minute they can read.

You can find the sight word lists for all grade levels on the district website at:

classroom.leanderisd.org/webs/elemla/reading.htm

You can use QRI passages or Fluency Probe Passages for tracking Reading Passage fluency. See your Language Arts Facilitator for the Instant Phrase Lists and the Fluency Probe Passages if you do not have them.
