Speech Program Services Descriptions
Houdini-- a walk-in speech student seen only by slp on campus for a limited amount of time. This student is usually not enrolled in any other programs.

PSLP (Preschool Speech and Language Program) Kim McCollum (Cypress)and Tenille Reichert(Mason) - Student with phonological processing issues needing more intensive treatment then Houdini- may also have a mild expressive or receptive language delay.

LEIP (Language Enrichment Intervention Program) Jennifer O'Connor (Naumann)A program for children with moderate language delays but little if any concerns exist in other areas. Class meets everyday for 2 hours and pairs with prek for typically developing peers enrichment and modeling.

ELE (Early Learning Environment)-Traditional PPCD classroom. Language rich in nature and containing typically developing peer models. Children in this environment may have a mixture of speech and language issues or may fall in the severe range on any one type of speech disorder. Speech services may or may not be a part of the child's program.

Please down load the LISD-PPCD flow chart and review evaluations and goals written to determine best placement for the 3-5 year old population.

Service Plan Children- May be seen outside of the regular school day for extra pay. Student is homeschooled or attending private school but qualifies with a speech Handicapping condition. Service plans are reviewed in the fall and run from September to the end of the school year.

This form is to be filled out when you evaluate a student who qualifies for services and is placed in sped and is between the ages of 3-5.

Parent notice that Slp will be giving instructional strategies for their child. Given out at RTI meeting once observation is requested
Ready Willing and Able sheet to continue child find evaluation
To be used when unable to get evaluation scheduled on a speech only evaluation. Must have documented several parent contacts and attempts to schedule.
Notice and Decision Ready willing and able letter
To be used when parents do not wish to avail themselves of the services of the district. An example might be when a student qualified for speech but prefer to persue speech outside of the school system
Speech screening form
This form is used for permission to screen at the speech screenings
Preschool teacher questionaire
Used to gather information from an outside preschool when evaluating a student

Form used to record therapy data to turn in for Medicaid

This form must be submitted hard copy to Amy prior to the 6th of the month.
